This 2-day foundation course is suitable for Health and Wellbeing Coaches, Social Prescribers, Care Co-ordinators, and other frontline roles in Primary Care Networks (PCNs), neighbourhood teams, and the voluntary/community sector, as well as anyone working in health and care who supports people to improve their health and wellbeing.
You will develop coaching skills that empower people to take greater control of their health and wellbeing by learning how to personalise your support. The course is grounded in personalised care principles and supports prevention, self-management, and improved patient experience.

Understand the model of personalised care, including roles, benefits, and impact
Explain wider and social determinants of health
Apply health coaching communication skills within personalised care conversations
Demonstrate these skills through reflective practice
2. Health Coaching Skills and Behavioural Insights
Define professional boundaries, expectations, contracting, and shared agenda setting
Explain the Stages of Change Model and behaviour change theory
Describe the COM-B model (Capability, Motivation, Opportunity, and Activation)

Delivered over a minimum of 8 weeks, this course combines the 2-day Foundation and 2-day Intermediate modules into one comprehensive learning journey. Learners may choose to complete both parts together or begin with the Foundation and progress to the Advanced module within 6 months creating a flexible skills escalator that supports real-world integration and reflection between modules.
